Enrollments
The participants`enrollment will have differentiated cost, according to the criteria described in the box below.

 * It is necessary to show the receipt in the event’s place.

PROCEDURES FOR ENROLLMENT

Fill in the online form below, choose the way of payment – identified deposit, credit card: VISA, MASTERCARD, AMERICAN EXPRESS or through Interest, only for Public Institutions.

• CREDIT CARD
  After filling in the enrollment form, choose the credit card you will use and follow the procedures indicated on the specific card’s page; Always wait for the finalization of the purchase.
  
  For the effectuation of the payment through credit card it is necessary that the computer is unblocked for this purpose, following the configurations below:
  
  In the Menu “Tools – Internet Options – Privacy ndash” should be in the MEDIUM level and the option Block Pop – Ups should be DEACTIVATED.

• INTEREST – only for Public Institutions

The enrollment with payment through interest will cost the same as the enrollment done in the place of the event, as it is generally released after the event.

After having finished filling in the data for enrollment, only the enrollment receipt will be generated, and it shall be handed in to the paymaster institution in order to direct the process of interest.

The paymaster institution shall send a copy of the Interest Note to the fax number 55-71 2102-6611 or to the e-mail address: inscricoes@gt5.com.br, with the following information:


Name of the Event: XXI CBEB
Name of the Registrant
Identification Number
Name of the person responsible for paying the Interest
Telephone Number and E-mail address

Information for the receipt: Name of Institution, Taxpayers Identification Number and Address.

After the payment, send a copy of the receipt to the fax number 55- 71- 2102-6611

ATTENTION: For the effectuation of the enrollment, it will be mandatory the sending of copy of the Interest, otherwise enrollment will not be concretized.

• IDENTIFIED DEPOSIT

After filling in the enrollment form, make the identified deposit at “Banco Real do Brasil”, Agency number 0896, current account number 0012401-6, favored: SOCIEDADE BRASILEIRA DE ENGENHARIA BIOMÉDICA, taxpayers identification number 26.468.709/0001-06. Send a copy of the receipt of deposit to the fax number: 55- 71 2102-6611.

 

** ENROLLMENTS IN THE VI FORUM OF TECHNOLOGY APPLIED TO HEALTH

The Event will be free for all the participants, provided that they enroll previously, in the established period of enrollment, from June to November/2008; It must be highlighted that the confirmation of enrollment will be determined by the limit of available vacancies, in the place where the VI FTAS will be held, once it has, at maximum, six hundred (600) seats.

Participants of VI FTAS will only have access to any activities from CBEB’2008 and CBECLINacute2008, if they are enrolled in these events.

General Information:

• Enrollments will ONLY be done through the Internet, in the event’s website.
• Participant will receive e-mails confirming the enrollment.
• The Secretariat of the Event will be at your disposal, since 8:00 a.m on November 16, 2008, for handing in the credentials to the pre-enrolled congressmen. in the Convention Center in Salvador
• If the registrant can not attend the Forum, the enrollment tax will not be returned, and the substitution of the congressman will only be allowed up to five (05) days before the Event.
